Shoulder Bullet Wound
Severe bleeding from junction, where the trunk meets a limb, (shoulder, armpit or groin) is best controlled by packing the wound with first aid gauze, a hemostatic dressing or any cloth such as a T-shirt.
Once packed, if possible, assume the CPR position, applying the most direct pressure until relieved by paramedics.
Gouge Wound
A gouge injury is when a portion of flesh is torn out, resulting in a deep cavity. Expect major blood loss. A tourniquet and wound packing is best for this type of injury in an arm or leg.
When a tourniquet is not readily available, pack the wound as tightly as possible and apply significant direct pressure. Although wound packing is possible for these injuries, the fact remains that a tourniquet is the best option.
Amputations
Catastrophic bleeding from amputated fingers, a hand or arm is definitely life threatening. Tourniquets are always the treatment of choice for these wounds. However, when a tourniquet is not available, direct pressure could be effective in controlling the bleeding.
Wound packing is not likely to be effective, however really tight bandaging will certainly help in controlling the bleeding.
Arm Bullet Wound
A person suffering severe trunk injuries, open or closed, are least likely to survive, unless they receive rapid surgical intervention. Junction injuries, where the trunk joins with a limb, can be treated with wound packing, increasing the chance of survival.
